A Flawless Horadric Gem asks for a frankly wild 25 million fragments of one colour, so it is easy to see why players are looking for shortcuts. It brings in an ambush enemy, often a nasty familiar face like the Butcher or X'Fal, and that kill can drop roughly 750,000 fragments. Chests add a lot as well. On Torment 12, a strong chest roll can hand over around 270,000 fragments. Finding both modifiers may take several failed escalations. Annoying, sure, but that setup is what makes the loop worth doing.

Do not take the exit portal either, because completing the dungeon shuts down the repeatable part. Instead, teleport to town, then enter a nearby ordinary dungeon. Many players use Seer's Reach near Temis, though the important bit is that it must be a normal dungeon and it must remove the escalation return portal. As soon as you enter, open the Sanctuary map and travel back to the active Escalation Dungeon. If it worked, you will return to the same third floor with fresh chests and another ambush waiting. There is no reason to clear the normal dungeon; it just slows you down.
Plan for the costs after the fragments
A good session can produce 12.5 million fragments in about 17 minutes, with 25 million possible in well under an hour when the loop behaves. That is a massive jump from ordinary farming routes. Still, fragments are only one bill. You need 25 Grand Gems, then 25 Horadric Gems, before combining them into one Flawless version, and the crafting eats gold and Forgotten Souls. Keep an eye on those supplies while farming.
